New Account Information Sheet

If you would like to become a Relay Port Agency Customer you must firstly complete a new application pack which comprises:

  • New Account Information Sheet
  • Direct Representative Letter*
  • T-form Indemnity*(if you have been granted permission to use the Relay T-form guarantee)
  • Post VAT Accounting Letter* (Importers only)

* Please note:  Text must be copied onto the Importer/Exporter letterhead.
